Apply to join Miss Nay

This goes to Miss Nay to answer. Teachers decide who joins their own studio, so an application is not put through automatically — and nothing is created until they say yes: no account, and nothing on anybody's register.

Your name

For a dancer under 18 this is the parent or guardian, who holds the account.

Your email address Your phone number

Optional. It goes to your teacher so they can reach you about the application.

The dancer's name Is the dancer under 18? Anything Miss Nay should know

Optional. What they have danced before, what you are hoping for, a day that suits.

Enrolment consent

In enrolling this dancer I confirm that:

1. I am the account holder, and for a dancer under 18 I hold parental responsibility for them.
2. I consent to this dancer being enrolled with the teacher named on the enrolment page.
3. I understand that for a dancer under 18 this account stays in my hands: I hold the sign-in, I can always read the chat between home and studio, and any document a teacher requires is signed by me.
4. I understand data is handled as the privacy page describes, and I can ask for this account's data or its erasure at any time.

I confirm all of the above.
